Sometime in 2003, List member Morris Krok ("Morris Krok"
("mailto:essence@xxxxxxxx;) reprinted it and offered it for sale in
the US (post of 9/28/03). I haven't seen his name recently, so I don't
know if he's still on-list, but you could try that e-mail address and
see if it gets through.

I have one, and I might be able to either photocopy or scan it. I tried
a couple of times to re-create it, but fell foul of a problem printing
to legal size paper using TYP. Come to think of it, I think I did
re-create it once, using (irony of ironies) Word Imperfect. Let me see
if that is feasible.
