Hyphenation errors

I have encountered strange hyphenation errors in XyWin version
4.12 under Windows 3.1. For example, I get "telep-hone" in
text without a soft hyphen, despite a "tel-e-phon*e" entry
in my hyphenation file.
Could the problem be the "DF HV=6,2,3" setting in my XWSET.DFL
file, or is this a bug in the program? Does anyone more
knowledgeable have any ideas?
