I don't remember the details, but I think I used MDRV (reverse) and tinkered
with my printer file to get itals in III+. The printer file will allow you to
assign any effect to an existing MD designation, and I think you'll need your
printer's documentation to figure out the printer-specific codes. Once you
do, you can pattern the MDRV revision after MDBO, substituting the ital codes.
