Index-creation software

One of our authors recently asked if we could recommend any index-creation
programs. We used to be partial to NLCindex (developed at the Newberry
Library) but I can't find any recent references to it on the Web. Does
anyone know if it is still alive? Does anyone recommend any other programs
for creating book indexes?
