Sporadic problems with *.KBD defs under OS/2

I'm running Xy III+ (3.55) in a VDM under OS/2 Warp 4.0. Since
applying FixPak 9 to Warp I've noticed a sporadic problem with
multiple hot-key combinations defined in my *.KBD file not being
recognized. CTRL+ALT+T, for example, sometimes just puts a "t" at the
cursor instead of executing the program mapped to the key combination,
and SHIFT-ALT+T likewise sometimes just produces a "T" at the cursor.
This happens both in windowed and full-screen sessions.
Has anyone else noticed this? Better yet, can anyone suggest a
workaround?
