I'm no Xywrite expert (having only discovered it a few
months ago), but someone on the list was kind enough to send me the Xy3
manual. This is probably more information than you need, but what the hell.

You can mark text in three different ways:
a) Single Word. For each word you want included in the table of contents or
index:
Move the cursor to the point immediately following the word
Type: [F5]x3[ENTER]
Press: [F3]
Result: The X3 triangle is inserted in the text, marking the word that
precedes it. There must be no space (or tab) between the X3 marker and the
word.
(Note: Go to Expanded Display and make sure there are no mode commands -
such as ≪MDNM≫ - between the text and the X3 marker. If there are, move
the X3 marker next to the text.)
b) Entire Line. You can mark any _phrase ending with a carriage return_ -
such as a title on its own line. The whole phrase, from marker to carriage
return (up to 200 characters long including embedded commands), is picked
up. The cursor must either be at the start of a line or have a space (or
tab) ahead of it.
Put the cursor _before_ the phrase ending with a carriage return.
Type: [F5]x3[ENTER}
Press: [F3]
c. Any Phrase. You can also enter a phrase that doesn't appear explicitly
in the text (or is in the middle of a line)...
Type: [F5]x3 (the phrase)[ENTER]
When you have done indexing, you go to the end of the document (more
follows from manual):
Make sure the cursor is at the beginning of the line. Enter a command T1
through T9 for a table of contents or I1 through I9 for an index. In our
example, the text was marked with X3 and we want a table of contents, so we
will use T3.
Type: [F5]t3[ENTER]
Enter the two commands SR IX (to place marked text) and SR PN (to place
page numbers) in the order you will want them positioned on a line in the
table of contents. Also include the leadering (LD) between them, if you
wish, and any margin (LM, RM) or other format command...
Now that everything is set up, we have choice of two ways to output the
table of contents or index.
a: Extract and Save to a Separate File
If you wish to revise youf table of contents or index before printing it,
you may want to save it in a separate file.
Enter the appropriate command TX1 to TX9 (Extract Table of Contents) or IX1
or IX9 (Extract Index) - for our example we will use TX3, which extracts
only the text marked by X3. To extract text from CHAPTER and save it to a
file called CONTENTS:
Type: [F5]tx3 chapter, contents[ENTER]
b. Print the Source File and Table of Contents
To print the entire document including its table of contents, simply print
the source document (here, named CHAPTER). (Unlike Step a above, this
procedure does not save the table of contents or index to a file)
Type: [F5]type chapter[ENTER]
Help is accessed by entering HELP or ? on the command line and then
pressing [ALT][F9] to access the help screens.

Since I had such a good experience with my last index on XyIII+, I found
an old disk for that and have it running (v. 3.54, 1988). But I do not
have the book any more, and the Xy4 commands do not work. I can't even
remember how to get help! Can anyone tell me the commands for marking from
a list, extracting an index, and HELP? I recall seeing a discussion of the
virtues of XyIII+ vs. Xy4 in the archives, so I'm hoping some XyIII+
admirer can send this to me.
For the record, I made one large file of the whole book; it is 741,665
characters long. It has served me well for searching various words through
the whole book.
When I ran a short test list through it (only 10 words) to mark for
indexing, and then extracted the index, all the page numbers were correct.
However, CORRECT did bizarre things, and did not mark all the words. It
would skip over entire sections and not mark anything, then resume again.
I studied these sections and their surroundings carefully in expanded mode
and could not find anything that would cause this to happen. I thought at
first this was because the file was too large, and when I ran it on one
extract it marked everything, but then on another it didn't. It also
embedded an within a command, always at the same place, no
matter what I did.
I can get Xy4 to work for me with a lot of manual intervention, but I
would also like to try it with XyIII+.
