Start ==> Settings ==> right click on "Printers" ==> Open
(Alternatively, Control Panel ==> Printers)

Click on "File" up top ==> Server Properties ==> the "Forms" tab

Check the box that says "Create a New Form". Give it a descriptive name in the
"Form Description for:" box (usually the new size "X x Y"), then fill in the
blanks below (units/height/width; leave the margins at *zero*) -- and finally...

"Save Form"

Done
