Book formatting???

I know some of you have prepared camera ready copy for books by using
XyWrite alone. Would any of you who have done this be willing to
share your basic formatting/layout commands with the rest of us?
Making something look *good* as a book is not an easy matter. I'd
like to see what other people have done.
